General Practice/PCNs Opportunity to Employ Trainee Pharmacists

There are many largescale changes happening to the way in which pharmacists are trained and the implementation of these changes has led to a complete overhaul of the training foundation programme.

Regulatory changes in relation to foundation training has opened up the opportunity for General Practice/PCNs to employ their own trainee pharmacists.

More information is available on the HEE website a summary of the key changes relating to foundation pharmacist training and general practice can be found below:

They would be eligible for the training grant (£26,500 from 2025/26 onwards) and be responsible for paying the trainee’s salary for the duration of the training programme, providing supervision including access to a prescribing environment /DPP, securing access to a period of training in an environment that has a dispensary overseen by a GPhC registrant.
  • All recruitment of trainee pharmacists must be via the national recruitment system (Oriel). In order to employ a trainee in 2025/26 programmes must be entered into Oriel by 1st March 2024.

Changes to Locality Education, Training and Workforce Development Provision for General Practices - Action Required

There has in recent months been an increase in the range of GM-wide primary care training provision available for the General Practice workforce to access and the Locality Education and Training Team is involved in both the development and promotion of a number of these initiatives.  This has replaced the need for some of the previous locality level commissioned primary care education, training and workforce development offers.

Due to the current financial challenges faced across NHS GM a mapping exercise is currently being undertaken to identify all the available provision with a view to reduce duplication whilst ensuring that additional, prioritised, education and training needs can still be funded and made available within the locality.  The proposed programme will be tabled at Stockport GP Board to enable Board representatives to discuss this within their PCNs and Practices, providing feedback back into the Locality Team to determine the programme which will run from April 2024.  Some key principles have been agreed which we wanted to share with you at this stage as there is an impact on practices which will need actioning prior to March 2024, the most pressing being in relation to the Learning Management System which is used by a number of practices to access, record and show compliance of workforce mandatory training and to book locality funded education events.

Unfortunately, from April 2024, the current Virtual College Learning Management System will not be funded by NHS GM.  Instead, practices will be supported to access the elfh platform.   Elfh is an NHS England programme in partnership with the NHS and professional bodies. It provides all the previous mandatory training provision available on Virtual College and allows practices to pull off compliance reports for their workforce.  However, one key change that practices need to be aware of is that elfh will not automatically alert individuals when they need to update their training to stay compliant so this will be something that practices and individuals need to manage to ensure ongoing compliance. The Locality Education and Training Team will support the transition to the new system and further information will follow in relation to accessing and storing the historical training records and certificates. Alternatively, if Practices wish to continue using Virtual College and fund it themselves we can link you up with the Account Manager at Virtual College to enable you to take this forward. In addition you may have an alternative system that you choose to use in your practice, there is no requirement for you to transition to elfh if this is not your preferred option.

If you would like us to support the transition of your practice to the elfh system please complete the following initial actions:

  1. Bulk upload of your workforce –  Please see the registration section of the elfh guide to enable us to ‘bulk upload’ your workforce. You will need to complete and return one of following ‘bulk upload’ spreadsheets by Friday 12th January to : gmicb-sto.learningdevelopment@nhs.net
    1. Bulk Upload NO SMT Allocation
    2. Bulk Upload Including SMT Allocation
  2. Mandatory training allocation – all modules can be allocated to your workforce when their details are first uploaded onto elfh.  This means all SMT modules will be allocated at the point your staff details are bulk uploaded onto the elfh system irrespective of when they last completed that training and irrespective if the module is correct for their role.    In the elfh guide there is a summary table showing modules that are in the automatic allocation and which ones each staff member needs to complete.  You can also choose to automatically allocate the Oliver McGowan Autism and Learning Disability module to your workforce by completing the Oliver McGowan Autism and LD Training Allocation Form.

Please note that whilst the future training programme is in development, there is still a commitment to fund Basic Life Support training and Masterclasses will still take place 4 times per year with funded Mastercall cover.  Further details about the full package of support will be shared with you when confirmed.

You can delegate or manage the process in your practice as appropriate. Please share this communication with anybody you feel needs to be made aware at this stage.

Primary Care Knowledge Boost Podcast - The Art of Stuck Cases

A Framework for Managing Complexity in Primary Care – Doctors Lisa and Sara talk with Consultant Psychiatrist Dr Mark Spurrell about a framework that can be applied to the care of complex patients with multiple problems or difficulties. It is particularly useful at times where we feel stuck, we are struggling to help or move forward and care may be fragmented with lots of care professionals feeling similarly. The idea is to use a value based approach using the whole team to breathe new life into the scenario, create positivity and momentum and build on this to help generate better outcomes for patients that are tailored to their values. The idea is originally for the MDT approach of Mental Health work but it has been interesting to discuss the approach for General Practice Patients. Access this and other podcasts here – Primary Care Knowledge Boost | a podcast by Primary Care Knowledge Boost (pckb.org).

Asthma for Children and Young People - Competency Framework and Training

NHS England and partners have developed a competency framework for anybody working with children and young people with asthma.  The framework is aimed at both health and non-healthcare professionals across all aspects of a child’s life, including childcare providers, education providers, uniformed services, sports coaches, social services and health care workers.   The framework divides different roles into 5 ‘tiers’. A tier describes the level of care a person may be expected to deliver to a child or young person with asthma. The more involved the care, the higher the level of tier.  It is assumed that for healthcare professionals undertaking the training they will already have appropriate training and skills relevant in the care of children and young people. These courses are to provide additional training specifically relating to asthma and should sit alongside appropriate frameworks of training, supervision and mentoring as required.  The tiers are not profession specific, but rather describe the minimum required knowledge and skills any individual must possess to safely carry out their role in caring for a child or young person with asthma. Individuals should look at their own role and choose the tier most appropriate to the care they deliver.  Full details can be found in the  National Capabilities Framework and training to meet the first few tiers can be found here at e-lfh – Asthma (Children and young people) – elearning for healthcare (e-lfh.org.uk).

Physician Associates

Physician Associates (PAs) play an important role within multidisciplinary teams and are identified as an important role for expansion in the Long Term Workforce Plan. Plan. PAs are being written into legislation to be brought into regulation by the General Medical Council. The timeline for regulation is currently Autumn 2024.

The Core Capabilities Framework for Medical Associate Professions sets a standard and offers the opportunity for PAs to develop and evidence their knowledge and skills, enabling the highest standards of practice as well as providing a structure for career and role development across a variety of settings. This framework sets out clear expectations about supervision at the different tiers of practice.

This is a useful tool for employers across primary care networks as it can be used to demonstrate that the PAs in their workforce meet the defined core capabilities required for the role and enable patient safety. It is also useful as a communication tool to support better patient and stakeholder understanding of the role of a PA in a multiprofessional healthcare team.

A 3-minute video explaining the Core Capabilities Framework can be found here – https://youtu.be/F8JE-TJxqxY

The framework should be used in conjunction with other guidance and frameworks, not limited to:

– General Medical Council (GMC) Good Medical Practice interim standards

– Advice for doctors who supervise physician associates and anaesthesia associates – GMC (gmc-uk.org)

An accompanying Career Development Framework (CDF) for the Medical Associate Professions (MAPs) is currently in development by national stakeholders. The purpose of the career development framework is to demonstrate to employers how MAP careers can progress, whilst enabling flexibility to develop career pathways at a local level ensuring professional identities can be developed and maintained. It is expected that the career development framework will support a reduction in differential career experiences and opportunities for MAPs, by ensuring a greater awareness and understanding of these roles for patients and the wider NHS, maximising their contribution in high quality safe services and ensuring sustainable growth to meet future workforce needs and capacity of service. This is expected to be published in March 2024.

Please email MAPS@hee.nhs.uk for any further information.

Sessions are designed to follow on from the Beat and NHSE Eating disorders e-Learning and were developed to help clinicians feel confident in identifying and treating eating disorders in people.   A Beat eating disorder clinician will present a 10-minute presentation on a related topic.   Following this clinicians will have the opportunity to discuss the topic in more depth, ask questions, share knowledge, consolidate learning and consider the application of knowledge to practice. To access the sessions, visit the Beat webpage. NHSE has also commissioned an eating disorders learning hub including top-tips sheets where you can find key trusted evidence-based learning curated and approved by MindEd’s expert panel. The sessions will take place 12pm – 1pm and include additional learning resources for those looking to increase their knowledge about eating disorders.

Care Navigation Training

The delivery plan for recovering access to primary care, published last month, committed to training 6,500 staff (one person from every practice in the country) in care navigation skills. Practice and PCN managers are now being asked to nominate one member of staff, ideally reception staff and care navigators involved in triaging requests, to complete the virtual care navigation training. Managers can book on nominated individuals now with sessions starting from 10 July. It’s anticipated that this will be a popular programme and that spaces will fill up quickly.  On completion, individuals will be confident to communicate effectively with patients and be able to signpost to the most relevant team member or local services depending on patient needs. An advanced training offer will be available at a later date for those with experience of care navigation who can support and embed it into practices and networks for sustainability and standardisation.  If you have any questions please email carenavigationtraining@england.nhs.uk.
