Pathfinder (Mastercall)

Mastercall provides an acute visiting/ hospital admission avoidance service for patients whose conditions has suddenly deteriorated (for example sudden onset of confusion) and if a clinician does not visit, could lead to a hospital admission.

The aim of this service is to keep people in their own homes and prevent unnecessary hospital admissions.  The service provides medical advice, treatment and urgent GP visits 24 hours a day, 7 days a week.

Care Home Pathfinder Service criteria includes:

  • Sudden onset of confusion
  • Suspected UTIs
  • Suspected chest infections
  • Acute onset of any infections (such as cellulitis) that has developed within the last 24 hours.
    (Please note this list is not exclusive)

 

The Care Home Pathfinder Service does not treat:

  • Ongoing medical problems
  • General deterioration
  • Chronic disease management
  • Sleeping problems
  • Any minor ailments that could be visited the next day as part of a routine visit
  • New/urgent mental health problems (follow existing procedures)
  • Medication queries.
