Mental Health Liaison Service for Older People

The service will work with members of the neighbourhood team to promote holistic assessment and care planning and to ensure that older people’s mental health as well as physical health needs are met.  The service is specifically designed to target older people with long term physical health problems and /or complex needs who are suspected of having dementia, delirium or depression or other mental health problems.

The mental health liaison service provides support and assessment for older people (aged 65+) and those with young onset dementia.

Referrals are accepted from members of the integrated neighbourhood teams (social workers, district nurses, ANP’s, therapy staff) and can be made by through faxing a referral to 716 4566.
