Type 1 Diabetes

SOCCER (Stockport, Optimising, Carbohydrate Counting, Education and Results)

SOCCER is a structured education programme that supports people to understand and manage their Type 1 diabetes. It promotes the skills necessary to estimate the carbohydrate in each meal and to inject the right dose of insulin.

The aim of SOCCER is to help people with Type 1 diabetes lead as normal a life as possible, while controlling blood glucose levels, hence reducing the risk of long-term diabetes complications.

SOCCER involves attending a 4-day training course (currently on 4 consecutive Tuesdays, 9am to 5pm) plus a follow-up session around 6 weeks after the course. This structured teaching programme is delivered to groups of 8 participants, and facilitated by trained educators, a Diabetes Specialist Nurse and Dietitian. The SOCCER course is about learning from experience. Over the four weekly sessions the participants will practice the skills of carbohydrate estimation and insulin dose adjustment.

Most of the training is built around group work, sharing and comparing experiences with other participants. However, there are opportunities for each person to speak to the SOCCER educators individually.

SOCCER allows people to fit diabetes into their lifestyle, rather than changing their lifestyle to fit in with their diabetes.

Referral Criteria for SOCCER Course

  • Type 1 Diabetes
  • 1 year post diagnosis and post honeymoon period
  • Age > 17 years
  • HbA1c < 108mmol/mol
  • Currently on a basal bolus regime
  • Willing to monitor blood glucose 4 times daily
  • Able to participate in a group session
  • Able to attend all sessions
  • Basic maths skills
  • Motivated to improve diabetes control
  • Absence of end-stage diabetes complications

Type 2 Diabetes

X-PERT Diabetes Programme

The X-PERT is a six-week group education programme for Type 2 diabetes.  It will help patients to understand more about diabetes and how they can manage it through lifestyle changes.  It is suitable for anyone newly diagnosed and those who have had diabetes for a while but want to learn more.

Each programme is run by 2 experienced health professionals who will guide attendees through all aspects of their diabetes.  It is a 2 ½ hour session each week for 6 weeks.  It is important that patients attend all the sessions to fully benefit from the course.

The X-Pert is run on different days in various venues around Stockport and evening courses are now available.

  • Week 1 – What is Diabetes?
  • Week 2 – Weight Management
  • Week 3 – Carbohydrate Awareness
  • Week 4 – Food labelling
  • Week 5 – Possible complications of Diabetes
  • Week 6 – Living with Diabletes and Planning for the Future
