Children’s Community Nursing Team

The Children’s Community Nursing Team (CCNT) provides family centred acute nursing care in the child’s own home.

The aims and expected benefits of the service are:

  • To reduce the number of children admitted to hospital.
  • To reduce the length of inpatient stay when admission is unavoidable.
  • To divert appropriate GP referrals to CCN Team by providing direct access to the team.
  • To divert GP referrals from the Emergency Department by providing direct access to the Team.

Currently the CCN team operates from

  • 8am – 9pm Monday – Friday
  • 8am – 6:30 pm Saturday/Sunday and Bank holidays.

It is expected that these hours will increase from 8am till 10pm, 7 days a week depending on need.

GPs can refer a child directly by phoning 07810 155181

Children who will be excluded from GP referrals to the CCN team are those who:

  • Are less than 3 months old and over 16 years.
  • Require URGENT medical attention.
  • First febrile convulsion.
  • If the GP has concerns around safeguarding.
  • If the family are anxious and frequently presents to the GP, but the child is not acutely unwell.
  • If home circumstances do not allow the child to be cared for at home.
