Steady in Stockport

As part of the Stockport Together programme, Stockport now has its very own falls prevention and bone health improvement service; Steady in Stockport. Here’s what you need to know about the service:

Who is Steady in Stockport for?

The falls service will work with older people who:

  • Had a near miss / first time faller
  • Want advice on falls prevention
  • Have loss of confidence / fear of falling
  • Need a full multi-factorial / environmental assessment and have not been seen by another rehabilitation service yet
  • Would benefit from osteoporosis management, bone health improvement and fracture prevention

The bone health service will work with people predominantly over 50+ who:

  • Have had a fragility fracture
  • Would benefit from support with medication management regarding osteoporosis treatment

What does the service offer?

The service focuses on falls prevention and bone health improvement, and offers

  • Multi-factorial falls assessments
  • education, public awareness and drop in sessions
  • one to one therapy interventions
  • bone health assessments (not until February 2018)
  • osteoporosis treatment in the first 12 months after diagnosis (not until February 2018).

Who makes up the team?

The Steady in Stockport team includes fracture liaison nurses, an occupational therapist, a physiotherapist and support workers, and has close links with hospital consultants.

How do I refer to the service?

To make a referral, complete the referral form below and send this to snt-tr.steadyinstockport@nhs.net.

(Please note this document can be uploaded directly into your EMIS system so that fields will pre-populate)

Please note that you should not make a referral if:

  • the person is already been seen by another rehabilitation service, for example Active Recovery, the Adult Community Therapy Team, or the hospital specialist Falls and Fracture Service
  • an urgent response is required. In this instance, contact the Crisis Response Team.
