Active Recovery Team

The Active Recovery Service is a short term rehabilitation service that offers holistic care built around the individual to support early and sustainable discharge from hospital, and support people to regain their independence.

Bed based services are also part of the Active Recovery service, recognising that some people need a phased return home. Marbury House in Heaton Chapel now has a number of step-down beds enabling people to continue their recovery and rehab in a supported environment.

With Transfer to Assess pathways in place, Active Recovery staff now assess people in their own homes or community setting, away from a busy hospital ward which is far better for the patient.

Active Recovery is made up of:

  • REaCH (Reablement and Community Home Support)
  • Intermediate Care – bed based service
  • Adult Community Therapy Team (ACTT)
  • Community Rehab workers
  • The Prevention Alliance (TPA)
  • Wellbeing Integrated Network (WIN)
  • Enhanced Care Support
