ix Disclose how many CHC assessments were performed by Stockport NHS Clinical Commissioning Group over the period 1st April 2013 to 31st July 2016 and how many persons qualified for CHC.

1929 assessments were performed, 415 qualified for Continuing Healthcare

Xiii Provide equality data collected on the distribution of CHC in the Borough of Stockport.
