The 2021-2025 Stockport Dementia Strategy builds on the implementation of Stockport’s earlier dementia strategies (2010 and 2017) and on the five themes of NHS England Dementia Well Pathway including relevant NICE guidance.
The new strategy highlights the achievements made so far in dementia care in Stockport, sets out the vision for 2025 and identifies key achievements by which the overall strategy will be measured.
