CCG FOI 630 – Physiotherapy & Rehabilitation

1.Please confirm/deny whether there is a community rehabilitation service in your area for treating the following conditions:

– Hip fracture
– Stroke
– COPD

Yes we currently have a community rehabilitation team for general rehabilitation  such as hip fracture, as well as a neuro rehabilitation team who also see Stroke patients.

We also have a  COPD  service

2.Please provide the patient pathway for accessing any services confirmed in request one.

Patients can access the services through referral from acute services, their GPs or other services

The community rehab service accept self-referrals

3.Please provide the waiting times for accessing any services confirmed in request one.

Waiting time for community rehab is 2.5 weeks

The neuro rehabilitation team have wait times of  5 – 15 working days depending on category of stroke.

There are no real wait times for the COPD service, patients are seen by the COPD nurses when identified.

4.Please confirm/deny that patients can be a) re-referred into or b) self-refer back into any services confirmed in request one. Patients can be re-referred

5.Please confirm the staffing make up of any services confirmed in request one.

The Neuro rehab team have staff consisting of :

1.63 x Occupational Therapist

2 x Physiotherapist

0.48 x Speech and Language Therapists

1 x Therapy Assistant

0.8 x Admin

The community rehabilitation team have staff consisting of:

1 x clinical services manager

1 x clinical lead

2.27 x Community Occupational therapists

6 x Community physiotherapist

3 x Assistant practitioners

1.14 x admin support

The COPD team has:

0.8 x  Service lead

5.89 x specialist nurses

1.24 x  physiotherapists

1.43 x admin

1.0 x band 4 & 5 assistant practitioner/exercise facilitator

0.21 x GPwSI

0.11 x Pharmacist

6.Please confirm/deny that patients can access MSK physiotherapy through each of the following pathways:
By seeing a GP who arranges the referral to the physiotherapy service Other-  referral from ICATS

7.Please confirm/deny that patients can access physiotherapy for their existing long-term condition through each of the following pathways:
