To manage patients with chronic disease, running specific clinic sessions and maintaining robust call and recall systems.
Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule. Provide travel health advice and administer appropriate vaccinations. The post holder is responsible for the delivery of general practice nursing to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long term condition, and the provision of preventative health care to the practice population. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. Participate in Quality Outcome Framework (QOF) and Extended Primary Care Service objectives Perform searches and audits Prepare for CQC inspections They will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures, Treat To maintain own annual registration with the NMC and adhere to Code of Professional Conduct. To maintain a safe and therapeutic environment for all PHCT staff, patients, carers and co-workers from other agencies. Ensure all statutory regulations (e.g. Health & Safety, COSHH, etc) are implemented and adhered to. Any other reasonable duties that may be required from time-to-time.
