The Care Quality Commission has published its findings following a local system review of Stockport. The full report can be viewed here: www.cqc.org.uk/localsystemreviews.
This report is one of 20 targeted reviews of local authority areas looking specifically at how people move through the health and social care system, with a focus on how services work together.
The reviews look at how hospitals, community health services, GP practices, care homes and homecare agencies work together to provide seamless care for older people living in a local area.
The CQC highlighted that the vision, ambition and plans for the future of joined-up health and social care for Stockport is the right one to meet the needs of our communities.
The CQC also praised the quality of care given by health professionals and the work being done to help support patients in the community and safely stay out of hospital. The care and support for people living with dementia in the community was also commended. In addition, during the feedback from the CQC inspection team they highlighted the quality and commitment of local GP Practices.
The report reflects that there is a commitment from staff at all levels across Stockport to provide person-centred care, reduce isolation and to empower people to make decisions about their care and support needs.
However, we recognise there is more to do and that there is a need to increase the pace of change to improve services and experiences for patients. All the partners across Stockport are working together already to address the areas for improvement and there is a strong commitment to make this happen.
