If you want to ask us for information which we may hold about you personally, this will be dealt with as a Subject Access Request under the terms of the Data Protection Act (2018). This could include information relating to complaints, funding decisions, health records, employment and training records.

The CCG reserves the right to charge a reasonable fee for photocopying/postage if the request is excessive and will notify the requester of the charges prior to completing the request.
