Introduction

NHS Stockport CCG is committed to being transparent and open in our decision-making.

To help members of the public understand who we are, what we do and how we take decisions about the local NHS budget we will routinely publish:

Fair Processing Notice

This Privacy Notice is part of our programme to make transparent the data processing activities we are carrying out in order to deliver on our commissioning activities.

This Privacy Notice tells you about information we collect and hold about you, what we do with it, how we will look after it and who we might share it with.

It covers information we collect directly from you or receive from other individuals or organisations.

Freedom of Information (Public Information)

Members of the public have a right to request information held by publicly funded organisations, like the NHS, under the Freedom of information Act (2000).

Under the FOI Act we have 20 working days to provide the information you ask for.

Subject Access Requests (Information about You)

If you want to ask us for information which we may hold about you personally, this will be dealt with as a Subject Access Request under the terms of the Data Protection Act (2018). This could include information relating to complaints, funding decisions, health records, employment and training records.

Under the Data Protection Act 2018 we have one calendar month to provide the information you ask for.

The CCG reserves the right to charge a  reasonable fee for photocopying/postage if the request is excessive and will notify the requester of the charges prior to completing the request.

Please ensure that you:

• specify the kind of information you wish to receive (e.g. Continuing Healthcare records, complaints);
• enclose proof of identity & address (a photocopy of your driving licence or passport and a utility bill);
• if you are requesting information about someone else, you will need to provide a signed letter of consent from the individual, proof of guardianship or power of attorney;

What if you are unhappy with our response?

Under the terms of the legislation, there is some information which we cannot give out to members of the public. If you are unhappy with any aspect of the way in which we deal with your Subject Access Request of Freedom of Information request you may complain, in the first instance, to the Information Governance lead through the address and contacts above.

If you are not content with the outcome of your complaint, you may apply directly to the Information Commissioner for a decision. The Information Commissioner can be contacted at:
