Stockport Together Update

Please find below a short update on the progress of activity within Stockport Together this week from Programme Director Tim Ryley:

 

  • The big news is that Stockport has been awarded £19m of £20m we asked for. This is thanks to a lot of different people, from BI and Finance, to the programme management teams. Our success is testament to the fact that we have been working on this partnership for the last 2 years.

The funding will now be used to implement the health and social care transformation plans as set out by the partner organisations. Having secured the investment to take this forward means we can now start to bring about improvement that will result in better care and experiences for the people of Stockport.

 

  • There will now be a big focus on two main items of work:

1.  The FT board, CCG Governing Body, Overview and Scrutiny Committee will each be reviewing the overview business case this week. This document describes 9 other more detailed business cases, showing how they each fit together.

 

The detailed business cases still have more work to be done, and this will need to be completed during August and September in order to release the £19m.

2.  Now that we have the money from GM and we know that the service models are heading in the right direction, we have to work up detailed plans in time for implementation to begin from October.

 

  • The Outcomes Framework team hosted a meeting earlier this week to identify how activity will now progress.

The Outcome Framework enabler is looking at how the health and care system can better work together to achieve better outcomes for people. In order to do this we need to understand what ‘good’ looks like, and be able to measure the impact we are having on people’s health, care and wellbeing.

To do this, a series of Expert Reference Groups have been established. The first set of meetings has been run to bring together groups of patients who represent different cohorts of the population, and follow up sessions are planned.

 The sessions to date have been clinically led sessions, which have facilitated really good engagement as we work through the development of the outcomes

  • Crowdsourcing will be launching shortly to get provider staff engaged with the Stockport Together activity. It has been developed to help us to engage frontline staff to develop the plans.

 

  • A meeting was held last week to outline the neighbourhood model and to engage with GPs, District Nurses and Social Work staff.  We have been working with Viaduct to enhance primary care involvement with the Stockport Together. This has been led by the providers, but will form the basis of putting the neighbourhood leadership structures in place.

 

  • The team working with NHS England on the rapid testing activity has received feedback following the first 25 days of activity. Michael MacDonnell provided really positive comments about the progress that’s been made. He commended the team for having launched clinics after 25 days, which is unique and a very positive step. Jeremy Hunt is very interested in hearing more about Consultant Connect, and wants to speak with a representative from Stockport.

 

  • The Integrated commissioning board continues to develop its work plan in order to establish the integrated commissioning arrangements.
