Do you know someone working in a care home or home care organisation that deserves some recognition for the great work they are doing?
Do you visit a care home that you think deserves an award for the excellent care and service they provide?

The awards aim to recognise the value of staff working in care homes and home based care.
There are 9 categories of award:

The closing date is the 14th December and nomination forms should be emailed to stockport.star@nhs.net
