Practice Community Champions

Following the successful introduction of the Practice Community Champions programme in Alvanley, Heaton Moor, Bracondale and Cheadle, the Stockport Public Health team are now available to provide support for four more practices in the remaining neighbourhoods of Tame Valley, Stepping Hill, Marple and Bramhall.

The Practice Community Champions are volunteers who can support the health and wellbeing of patients in a variety of ways, from promoting bowel screening to helping socially isolated people to get involved with community activities, or helping people to increase their physical activity through walking groups, as agreed with the Practice.  This can help reduce inappropriate demand for GP and other clinical appointments.

The support programme from the Public Health Team includes working closely with each Practice to recruit and train a team of volunteers, who will receive induction and training workshops, delivered with direct input from the practice.
