Palliative Care BiteSize

The Stockport Palliative Care Team has provided a bitesize guide for Primary Care to support colleagues when  “Diagnosing Dying”

  1. Review the patient!
  2. Agree with another health care professional (usually a District Nurse) that this person is now dying.
  3. Document the discussion – preferably in the Individualised Plan of Care document or District Nursing notes in the patient’s home.
  4. Then (and only then) write up the authorisation sheet for the anticipatory medications
  5. Complete Adastra for Mastercall
  6. Ensure an up to date DNACPR is in place
