Four mental health liaison practitioners have started work within the integrated neighbourhood teams. The neighbourhood liaison practitioners come under the management of the wider Older Peoples Mental Health Liaison Service based at the Meadows and therefore have access to mental health resources and the older peoples CMHT at The Meadows.
The service aims to work with members of the neighbourhood team to promote holistic assessment and care planning and to ensure that older people’s mental health as well as physical health needs are met. The service is specifically designed to target older people with long term physical health problems and /or complex needs who are suspected of having dementia, delirium or depression or other mental health problems.
The mental health liaison service provides support and assessment for older people (aged 65+) and those with young onset dementia. Referrals are accepted from members of the integrated neighbourhood teams (social workers, district nurses, ANP’s, therapy staff) and can be made through faxing a referral to 716 4566.
