Smoking Cessation Q2 Reminder

This is a gentle reminder that your practice quit attempt returns for Quarter 2 are due back to ABL health by Friday 23 November 2018.

Quarter 2 returns are for quit attempts set during the period of 1 July 2018 to 30 September 2018.  These need to have been followed up and an outcome recorded on the appropriate form.  Copies of the form can be sent back in paper version or electronically.
