The crisis response team (CRT) is appropriate for adults who do not currently need acute hospital intervention but are at risk of hospital admission within 24 hours if no effective or appropriate urgent response is available within the neighbourhood.
The CRT will respond to a health and/or social care ‘crisis’ by dispatching the most appropriate team to a person in their usual place of residence. It will aim to provide the response within 2 hours.
Whilst the service is for all adults (>18 years), it is anticipated that referrals will be predominantly for people aged >65 years. The service will operate 7 days a week between the hours of 8am and 10pm.
The aim of the CRT is to enable, promote and maintain independence and provide a safe alternative to hospital admission where this is in the best interests of the person. The CRT has direct access within the new Intermediate Tier Service to other health and social care provision.
The CRT will accept referrals from health and social care professionals, for example, GPs, community nurses, therapists, social workers, Emergency Department.
Access to the CRT will be through the new Intermediate Tier Hub. Health or social care professionals wishing to make a referral should call 0161 476 9665. The call will be answered by a call handler, who will record the person’s demographic details. The call handler will then transfer the call to a skilled health or social care member of staff, who will be able to triage and agree the appropriate service response.
The aim is to ‘warm’ transfer calls whenever possible in order that the referrer and CRT professional can discuss and agree the most appropriate response.
