Target Audience/Eligibility Criteria
This session is targeted at GPs and all NHS Staff across all settings who have care responsibilities for people with life limiting long term illnesses and those who are dying.
Course Aim
The aim of the course is to improve end of life care delivered across healthcare settings.
Course Content
The course content will include:
- Recognition of decline using Proactive Identification Guidance.
- Introduction to Advance Care Planning
- High quality care of the dying person.
Objectives of this Training
Non reversible decline will be recognised.
- Patients will be given the opportunity to think and plan ahead by being involved in decisions about their care
- Advance care planning will improve patient outcomes
- Dying people and those closest to them will receive care that reflects national guidance

This course is free of charge for all those who work in Stockport General Practices, for all other delegates the cost is £50 per person.
