These sessions are suitable for all health and social care Professionals involved with people with life limiting illnesses.

Course Aim

Stockport Specialist Palliative Care Team are offering advance care planning training, this training includes why advance care planning is important, how to start conversations, where to record and how to share care plans.

Only delegates who have previously attended the Advance Care Planning course should attend the  End of Life and QoF Quality Improvement Points session
