Getting Started with Long Term Condition Assessment and Management in Primary Care – COPD

Target Audience/Eligibility Criteria

This course is suitable for those new to General Practice OR those currently working within General Practice who have not previously been involved in a specific disease area OR those who are looking to update their knowledge and understanding.

Course Aim

This is part of a suite of LTC modules. COPD is the second module. It can be undertaken as a standalone module or as part of the full programme. This module aims to provide a sound educational base in the assessment and management of people with COPD and teach General Practice Nurses how to carry out safe and effective clinical reviews in people diagnosed with COPD.

Course Content

This course will cover:

  • Pathophysiology of COPD
  • Aims of COPD Management
  • Purpose and Structure of the COPD Patient Review

Learning Objectives

  • At the end of this module delegates will be better able to:
    Carry out an effective patient specific COPD patient review that is in line with current evidence based national guidelines and that will identify risk factors for future adverse outcome
  • Construct and implement a patient specific management plan that will reduce future risk of adverse events
