CVD – Chronic Disease Assessment and Management in Primary Care

Target Audience/Eligibility Criteria

This course is suitable for those new to General Practice OR those currently working within General Practice who have not previously been involved in a specific disease area OR those who are looking to update their knowledge and understanding.

Course Aim

This is part of a suite of CD modules. CVD is the second module.  It can be undertaken as a standalone module or as part of the full programme. You will gain an understanding of the critical importance of accurate clinical history taking and will learn how to carry out detailed disease specific patient reviews. You will learn how to analyse data collected at review in such a way as to define current level of disease control & predict the future risk of adverse outcomes

  • CVD Risk Assessment
  • Hypertension
  • Angina
  • Atrial Fibrillation
  • TIA and Stroke
  • Heart Failure
  • Chronic Kidney Disease

Learning Objectives

At the end of this module delegates will be better able to:

  • Identify a patient with poor control of CVD and future risk of adverse outcomes
  • Discuss the latest national & international disease specific guide recommendations
  • Construct & implement a disease/patient specific & evidence-based management plan
  • Evaluate & adjust disease specific management plans based on follow up review data

These sessions will be carried out via a video conferencing platform called MS Teams, delegates will need a computer or smart phone to join, where possible they are encouraged to use a web camera to fully participate. All joining details and coaching will be available before the session.

Please note ALL four sessions must be attended
