The Stockport Palliative Care Team has provided a bitesize guide for Primary Care to support colleagues when\u00a0 \u201cDiagnosing Dying\u201d<\/p>\n

    \n
  1. Review the patient!<\/li>\n
  2. Agree with another health care professional (usually a District Nurse) that this person is now dying.<\/li>\n
  3. Document the discussion \u2013 preferably in the Individualised Plan of Care document or District Nursing notes in the patient\u2019s home.<\/li>\n
  4. Then (and only then) write up the authorisation sheet for the anticipatory medications<\/li>\n
  5. Complete Adastra for Mastercall<\/li>\n
  6. Ensure an up to date DNACPR is in place<\/li>\n<\/ol>\n
