{"id":6441,"date":"2017-12-08T13:09:48","date_gmt":"2017-12-08T13:09:48","guid":{"rendered":"http:\/\/www.stockportccg.nhs.uk\/practicehub\/?page_id=6441"},"modified":"2019-02-19T12:45:28","modified_gmt":"2019-02-19T12:45:28","slug":"childrens-community-nursing-team","status":"publish","type":"page","link":"https:\/\/www.stockportpracticehub.co.uk\/practicehub\/local-services\/childrens-community-nursing-team\/","title":{"rendered":"Children&#8217;s Community Nursing Team"},"content":{"rendered":"<p>The Children\u2019s Community Nursing Team (CCNT) provides family centred acute nursing care in the child\u2019s own home.<\/p>\n<p>The aims and expected benefits of the service are:<\/p>\n<ul>\n<li>To reduce the number of children admitted to hospital.<\/li>\n<li>To reduce the length of inpatient stay when admission is unavoidable.<\/li>\n<li>To divert appropriate GP referrals to CCN Team by providing direct access to the team.<\/li>\n<li>To divert GP referrals from the Emergency Department by providing direct access to the Team.<\/li>\n<\/ul>\n<p>Currently the CCN team operates from<\/p>\n<ul>\n<li>8am &#8211; 9pm Monday \u2013 Friday<\/li>\n<li>8am &#8211; 6:30 pm Saturday\/Sunday and Bank holidays.<\/li>\n<\/ul>\n<p>It is expected that these hours will increase from 8am till 10pm, 7 days a week depending on need.<\/p>\n<p><strong>Referrals<\/strong><\/p>\n<p>GPs can refer a child directly by phoning\u00a0<strong>07810 155181<\/strong><\/p>\n<p>Children who will be <em>excluded<\/em> from GP referrals to the CCN team are those who:<\/p>\n<ul>\n<li>Are less than 3 months old and over 16 years.<\/li>\n<li>Require URGENT medical attention.<\/li>\n<li>First febrile convulsion.<\/li>\n<li>If the GP has concerns around safeguarding.<\/li>\n<li>If the family are anxious and frequently presents to the GP, but the child is not acutely unwell.<\/li>\n<li>If home circumstances do not allow the child to be cared for at home.<\/li>\n<\/ul>\n<p><strong>Further information<\/strong><\/p>\n<ul>\n<li>[Download not found]<\/li>\n<li><div class=\"dl-template\">\n    <a class=\"download-link filetype-icon filetype-pdf\"\n       title=\"\" href=\"https:\/\/www.stockportpracticehub.co.uk\/practicehub\/download\/6445\/?tmstv=1776470342\" rel=\"nofollow\">\n        CCNT Operational Policy    <\/a>\n<\/div><\/li>\n<\/ul>\n   ","protected":false},"excerpt":{"rendered":"<p>The Children\u2019s Community Nursing Team (CCNT) provides family centred acute nursing care in the child\u2019s own home.
